Flat foot surgery

I am a 37 yom. I've been referred to see a surgeon about possible flat foot surgery. I was born with flat feet, that have gotten really bad and hurt pretty much every day, all the way up to my lower back. Also, my achilles tendons are very tight and hurt every time I run, etc. I've been wearing custom orthotics for about 15 years and it's gotten to where they actually seem to make things worse. I understand that the surgery is extremely involved (they are recommending an "evans osteotomy with FDL transfer, and achilles lengthening). My questions are #1 can a surgery really create arches in flat feet, and #2 do most patients who undergo this really say it helps? Thanks for your reply. I'm VERY apprehensive about surgery, and would like ANY feedback I can get.
