Growth Plate Broke Off!

Dear Doc:
I am a 29 yr old male who has the knuckle of my left foot, second toe from the big toe, destroyed about 15 years ago (The growth plate broke off and everything is jagged). I think it was caused by overuse since I excelled at track, and although the docs told me to stop, i didn't. Now, I am starting to notice a gait change in my walk and running stride. Although I land normally on this foot, I push off on the extreme outside of my left foot , and I think it is starting to give me a left knee problem (on the outside).
My question is :
1. should i get the bone that broke off (growth plate) out of the joint as recommendend by a podiatrist a few years ago?
2. Are there orthotics out there for people who roll to the outside due to such injuries?
Any advice would be appreciated!
