Top Foot Pain

I am male 37 years old long distance runner. For several months I have been experiencing a pain on the inside top of my right foot in the location of the anterior tibial tendon and tendon that goes to big toe about where about where shoelaces are tied. The pain is sharp when foot is moved laterally and dorsiflexed with toes to shin. I sometime feel pressure and tingling at the top of the arch. I have always had tight hamstrings on that leg. I feel it is associated in some way. When I dorsiflex both feet, I have less range of motion in hurt foot as well as my big toe rises above other toes. Big toe on well foot stays even with other toes and does not move upward. I have not run for over 3 months and have been treated from physical therapist for "Anterior Tibial Tendinitis" which is not helping. I have been cross training with ellipticle and swimming and have been stretching hamstring. I have been using orthotics in shoes for a couple of years and have tried loosening shoelaces. Pressure and pain is also felt when standing on heel, possible due to dorsiflexion of foot. Your advice would be greatly appreciated.
