right ankle pain

I am having extreme outside right ankle pain. Had x-rays and they are clean. Having an MRI this Saturday. I am a very active 45 year old woman, exercise regularly and I am not overweight. The pain comes on unexpectedly and is debilitating. It lasts anywhere from 10 seconds to 30 minutes. I take tylenol which just takes the edge off. It doesn't make a lot of difference whether leg is elevated or not. This has been going on for 3 months. It came on very gradually,I had one sharp pain that stop me in my tracks, lasted maybe a minute or two, then it was gone and forgot all about it until the next time which was maybe a month later. Same thing, then left until next time and a few days ago is when it became more constant. It lasted throughout the night, waking up in extreme pain, then a few attacks throughout the day and again in the evening, but no more during the night while sleeping. There is really no pattern here of the cause.
Any advice would be appreicated. I cannot function due to the transient nature of the pain.
