Pain of top of foot

I have pain all the time on the top of my foot about where you would "tie" a shoe. I was told by one Dr. that my laces were too tight. I worked with that and have been wearing sandles all sumnmer. It still hurts. Even the sheets laying on it at night hurt. What is this?
