Late effects of hammer toe surgery

I had hammer toe surgery (removal of bone at the joint) on my right second toe over 20 years ago. I've had no problems with it since, until this week when it began to hurt intensely right at the post-surgical joint. I haven't bumped it or worn new shoes, or had any other changes. What could cause this, and what can I do about it?
