I searched the web for more info: the problem is caused (like you thought) from wearing shoes too small, and from the toe/toenail "banging into" the inside front tip of the shoe. Marathon runners get this alot. Supposedly the bruise/blood under the toenail will make the nail fall off, but another will grow back (taking a year to complete).

That's all I could find. It doesn't seem as serious as I first thought, so I'm not as worried.

By the way: I probably never would have responded back... but the part about your Doc Martens made me want to help! I still can't wear my Docs because of the tenderness of my toenails. My Skechers boots are big enough though!
