top of foot pain

I have fallen arches, and according to my chiropractor, fallen navicular bone and metatarsal heads in my left foot. I am wearing orthotics, but continue to have a pain most of the time toward the top of my foot, which increases when I flex my foot. Sometimes, it feels as if the muscle is contracted and I have difficulty standing of walking on it. The pain sometimes feels as if there is a pinched nerve. What should I do?
