The Sural nerve is a sensory nerve on the lateral side of the lower leg and foot. It can get trapped up in scar tissue. But that does not sound like why you have atrophy (shrinkage) of the calf muscle. That usually comes from disuse. It is common after an Achilles tendon injury. Have you had a recent MRI to see if the area has healed? Thickness is often noted permanently after a repair of a tendon.

You should gather as much information as you can so you are as informed as possible as to what the prognosis is.
