There could have been some ongoing repetitive strain that caused a weakening or focal necrosis of the plantar fascia. If it had been injected with an insoluable corticosteroid then that could have also weakened it.

When heel spur surgery is done, the plantar fascia is cut. Usually it is the medial and sometimes the central band. It has been discussed in the literature that if the lateral band is cut it can destabilize the lateral column of the foot.

Healing can be affected by many factors such as smoking, activity level, medications that are taken (such as anti-inflammatory medications or certain antibiotics) the list goes on.
