Swollen Left Ankle and Foot

My left ankle and foot have been swollen for a very long time now... years, even. It's become so normal for one ankle and foot to be swollen to nearly 2x the other foot that we refer to it as being my "fat kid foot." Sometimes it gets so bad that I cannot wear shoes w/laces... not a problem in the summer when I could wear flipflops, but now a major problem that it is winter. I've tried keeping it elevated, I've tried heat, I've tried cold, I've tried everything. My chiropractor didnt know what to do, and neither did my massage therapist. When my fiancee was rubbing it, trying to ease the pain of the swelling, he found a lump on the top of my foot, right by where the ankle-bone usually contours into the top slender part of the foot. Does anyone have any advice? All of my doctors dont know what it could be.
