Without examining you I can not be sure. From your description, it does not sound like gout. With gout there is always a red swollen joint. One of the more common probalems under the 1st metatarsal head is a fracture of either one or both of the sesamoid bones. If you have not seen a podiatrist yet, I recommend you do so.

A MRI might also be helpful.
