Without seeing your foot I can not say anything for certain.

However from your description you might have a metatarsal cuneiform exostosis. This is a slowly enlarging bone spur in the area you described that can irritate the overlying tendons and nerves. You certainly have described the irritating factors well.

As far as what to do- First I would recommend an x-ray,sometimes a diagnostic ultrasound to evaluate soft tissue inflammation.

Padding of the shoe is one option. Another is surgery to smooth down the spur.

I recommend you see a podiatrist for further evaluation.
