There can be a number of possible causes for his pain. Without examining his foot, I can not say anything for sure. Possibilities can include nerve entrapment, soft tissue cyst (ganglion) bone cyst, sinus tarsi syndrome.
Unfortunately, I can't tell you what to do about his problem. When it hurts him enough he will see a doctor. Preferably a podiatrist.
