Persistent instep pain despite orthotics

After years of instep pain while ice- and inline-skating (or skiing) I visited a podiatrist. Utrasound showed focal inflammatory patches along the plantar ligament and orthotics were prescribed.

It's been a year and I still can only skate a few laps without feeling the burning. When I run I still get shin splints too (in college I actually developed a stress fracture after playing soccer on shin splints). I'm extremely frusterated because I love to speedskate and skate inline marathons.

Are there any other possible conditions besides fallen arches?
