I think you are asking about a porokeratosis.

We don't really understand why these occur. They are quite painful and usually have a nerve attached underneath.

Treatments can include, trimming the lesions, injection of cortisone serial injections to sclerose (kill or shrink) the nerve underneath and even surgery.
