red, puffy, burning toes

I have been dealing with what was diagnosed by a doctor five years ago as a "fungal" infection from wearing furry lined boots without socks (very stupid). Over the five years and more prevelant in the winter months, my toes (all of them now) get very puffy, red and burning.This happens mostly at night. The toes are extremely tender and if I stub a toe on something it is very painful. I have tried most topical treatments and several herbal I am 59 and not a diabetic. I have no other health problems
treatments. Soaking in cool water helps a lot. I am sorry to report I do not have medical insurance and am trying to treat this myself. I will be most thankful for any advice.
