Excruciating Back of Heel Pain

I have been under a Board Certified Podiatrist care for the past 3 months due to extreme pain in the back of my right heel. I have been diagnosed with Plantar Fascitis, received poured orthodics, utilize night splint, completed 5 weeks of Physical Therapy and just received my 1st(very painful)cortisone shot as nothing else has seemed to help. The shot made my heel better for approx. 12 hours, now I am back to having pain. It feels like the back of my heel is going to explode outward, and the tendon running along the bottom of my foot feels like it is unraveling. I am concerned maybe I have Insertional Achillies Tendonitis which I plan to discuss with my Dr. I am scheduled for another shot in 2 weeks, but am very hesitant as it did not seem to help for more than a few hours and the damage that can be done (fat pad atrophy and/or Plantar Fascial Rupture). What can I do? This is starting to affect my well being in many ways such as loss of sleep and depression from the constant pain. I am about 30lbs overweight which I am diligently working to reverse. Please help:-(
