tip of toe and flexor hallucis longus

Hi! I have a question....Firstly..I have crepitition beside the med malleolus when big toe is plantarflexed and moving from flexion to extension....I assume it is the muscle tendon listed in the subject line(I am not a dancer).

My main concern however, is the thickening @ the tip of my big toe (same foot as crepitition). It looks simply like a callus (MD agrees) but I wonder what causes pressure there!! I also pronate on that ankle. I wonder if the concerns are all related, as (R) knee/hip are troublesome also.

So to summerize, has anyone ever heard of thickening at the tip of that large toe before...any known causes?
