Without seeing and evaluating the situation, it is merely a guess. One possibility is some type of rheumatoid arthritis (possible psoriatic arthritis which we look for the classic "sausage toes".) The arthritis specialist will likely run some very specific blood tests that may be helpful in aiding the diagnosis.
